The Wellness Coordinator is responsible for the supervision, provision, and quality of supportive services in Assisted Living.  The Wellness Coordinator is responsible for the procurement of appropriate supplies and equipment and operating within the departmental budget.  The Wellness Coordinator is responsible for the selection, training, discipline and supervision for all non – nursing department personnel. 

Tasks may include, but are not limited to the following:

  • Handles all medication related issues to ensure compliance.  Ensures medication carts are in compliance.  Controlled medication are accounted for per policy including oversight of shift to shift counts and narcotic count sheets.
  • Responsible for all self-medication assessments and monthly oxygen assessments.
  • Daily monitoring of medication compliance by running Yardi and Viewmaster reports and ordering medications from the pharmacy. 
  • Communication with Med Techs on medication availability and re-ordering.
  • Must immediately report any events to Regional/Corporate staff per the Reportable Events Checklist Policy.
  • Participates in the coordination of supportive services through departmental staff meetings. 
  • Maintains clinical skills and stays knowledgeable of current standards of nursing practice through nursing education related to geriatrics.
  • Assists residents and families by offering support in dealing emotionally with the aging process and illness.
  • Fully utilizes the YARDI electronic health records program to manage resident care tracking and optimization.
  • Ensures compliance with all regulatory requirements including corporate compliance and HIPAA Privacy Standards.
  • Medication reviews with Residents and family when requested or warranted.
  • Reviews and corrects any noted follow up from Pharmerica pharmacy or clinical reviews.
  • Attends social functions, as appropriate.
  • Oversees ordering of all medications, contacts physician’s offices to request refill, notifies family for pick up, to ensure medication are available and maintain compliance with written physician orders.
  • Possesses ability to communicate effectively with residents, families and staff.  Must use sound judgment, works well under pressure and possesses strong organizational skills.

 Qualifications:

  • Graduate of an accredited school of nursing.  Licensed Vocational Nurse or Registered Nurse with a current license in the state of Texas.
  • Two years’ experience in geriatric nursing and/or B.S. Degree in nursing.  Participation in professional organizations, dealing primarily in geriatrics preferred.  Minimum of two years nursing administration experience, preferably in a long-term care setting. 
  • Computer proficiency.  Must be comfortable utilizing technology systems on a daily basis to manage resident care and tracking.
  • Strong interpersonal and communication skills

Skills and Abilities: 

  • Must possess the ability to communicate effectively with all levels of management, employees and outside contacts.
  • Possess the ability to make independent decisions when circumstances warrant such action.
  • Possess the ability to seek out new methods and principles and be willing to incorporate them into existing practices.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions through data analysis.
  • Willingness to work beyond normal working hours, when circumstances dictate the need.
